Safety and Media Consent
For the safety and well-being of all students, parents and guardians agree to follow the academy’s policies and provide accurate emergency contact information. Parents or guardians remain responsible for supervising their child before and after class.
I understand that photographs and videos may occasionally be taken during classes or events for educational and promotional purposes. By completing this registration form, I grant permission for the academy to use photos or videos that may include my child on its website, social media, or promotional materials.
If you prefer that your child not appear in photos or videos, please notify us in writing.
